To apply for CACHE membership, fill out the form below. Before applying, please make digital copies (photos or PDFs) of your training credentials. You will need to upload the following immediately after receiving an email from us:
- Your hypnotherapy training credentials, including school(s) attended and course syllabus (syllabi)
- Proof of successful examination(s)
- Proof of current, valid practitioner insurance
Once approved, your membership payment will be processed. You will then receive an email with login information giving you full access to the members area of the CACHE website. Your membership certificate will be mailed to you.
